ORLANDO, Fla. - October 23, 2011 Two new studies add to the growing body of evidence that a new approach to cataract surgery may be safer and more efficient than today's standard procedure. The new approach, using a special femtosecond laser, is FDA-approved, but not yet widely available in the United States. It's one of the hottest topics this week at the 115th Annual Meeting of the American Academy of Ophthalmology. Research reported today by William W. Culbertson, MD, of the Bascom Palmer Eye Institute at the University of Miami School of Medicine, and by Mark Packer, MD, of Oregon Health and Sciences University, confirms several advantages of laser cataract surgery.

Laser Lens Fragmentation Boost Safety by Reducing Need for Ultrasound

Dr. Culbertson's team studied how pre-treating cataracts with the femtosecond laser affected the level of ultrasound energy needed to soften the cataracts. This emulsification is performed so that the cataracts can be easily suctioned out. Surgeons want to use the lowest possible level of ultrasound energy, since in a small percentage of patients it is associated with slower recovery of good vision after surgery and/or problems with the cornea, which is the clear outer layer of the eye. Ideally, in appropriate cases, ultrasound use would be eliminated altogether.

In Dr. Culbertson's prospective, randomized study, 29 patients had laser cataract surgery with a femtosecond laser in one eye and the standard cataract procedure, called phacoemulsification, in the other. Laser surgery included: a laser capsulotomy, which is a circular incision in the lens capsule, followed by laser lens fragmentation, then ultrasound emulsification and aspiration. Lens fragmentation involved using the laser to split the lens into sections and then soften it by etching cross-hatch patterns on its surface. Standard surgery included a manual incision, followed by ultrasound emulsification and aspiration. After cataract removal by either method, intraocular lenses were inserted into eyes to replace the natural lens and provide appropriate vision correction for each patient.

The use of ultrasound energy use was reduced by 45 percent in the laser pre-treated eyes compared with the eyes that received the standard cataract surgery procedure. Also, surgical manipulation of the eye was reduced by 45 percent in eyes that received laser pre-treatment as compared to manual standard surgery. This study involved the most common types of cataracts, those graded 1- 4. Dr. Culbertson notes that these findings may not apply to higher grade cataracts.

"In clinical practice, surgeons would expect safer, faster cataract surgery when laser pre-treatment is performed before cataract removal," said Dr. Culbertson. "The combination of precision and simplification that is possible with the femtosecond laser represents a major advance for this surgery."

Laser Lens Fragmentation Protects Corneal Endothelial Cells

Dr. Packer's team at the Oregon Health and Sciences University in Portland, Oregon, assessed the safety of laser cataract surgery in terms of loss of corneal endothelial cells, as measured after cataract surgery. Measuring endothelial cell loss is one of the most important ways to assess the safety of new cataract surgery techniques and technology. These cells preserve the cornea's clarity, and since they don't regenerate, they must last a lifetime. Dr. Packer's study found that when laser lens fragmentation was used in 225 eyes, there was no loss of endothelial cells, while the 63 eyes that received standard treatment had cell loss of one to seven percent.

"Our finding, that laser lens fragmentation appears to protect corneal endothelial cells, represents a significant benefit of this new surgery," said Dr. Packer. "This procedure is safer than standard cataract treatment and is likely to mean better vision and fewer eye health concerns for cataract patients, over the long term."

Earlier studies of femtosecond laser cataract surgery found other benefits. The laser allows the surgeon to make smaller, more precise incisions and to perform improved capsulotomies, which is the removal of part of the lens capsule that make intraocular lens (IOL) placement more secure. This reduces the chance that an IOL will later become displaced. Also, laser cataract surgery appears to improve results in patients who opt for advanced technology IOLs, plus corrective corneal incisions, to achieve good all-distance vision.

Femtosecond lasers have been used by ophthalmologists for years in refractive surgery such as LASIK, in-corneal transplants, and in other procedures. In 2009, a new type of femtosecond laser that could reach deep enough into the eye to be used in cataract removal was approved by the FDA. APNewsBreak: Banks nowhere near deal on Greece (AP)

BRUSSELS ? A top bank lobbyist insisted Saturday that banks and the eurozone are far from reaching a deal to cut Greece's debt, despite claims by eurozone finance ministers that they will ask banks to take steeper losses on their Greek bonds.

Although the ministers did not say how much of a cut they are aiming for, a report from Greece's international debt inspectors suggested that the value of Greece's bonds may have to be slashed as much as 60 percent to get the country solvent enough to repay its debt.

The ministers on Saturday sent their chief negotiator, Vittorio Grilli, to re-start discussions with banks and other private investors on a new deal for Greece.

However, Charles Dallara, the managing director of the Institute of International Finance, who has been leading the negotiations for the banks, said in an interview with The Associated Press that an agreement remained elusive.

"We're nowhere near a deal," he said.

Banks in July agreed to accept losses of about 21 percent on their Greek bonds. However, eurozone leaders have since reopened the deal and Greece's international debt inspectors ? the so-called troika of the European Commission, the European Central Bank and the International Monetary Fund ? say Greece's economic situation has deteriorated dramatically since the summer.

In a report Friday, the inspectors said that under the July deal, Greece would need an extra euro252 billion ($347 billion) in loans from the eurozone and the IMF ? on top of the euro110 billion ($152 billion) it has been relying on to pay bills since May 2010.

But Dallara said new plans to slash Greece debt would still leave the country as "a ward of Europe" for years.

He declined to say how much in losses banks would be willing to accept, saying only "we would be open to an approach that involves additional efforts from everyone."

Dallara was in Brussels, where eurozone finance ministers have been meeting for two days of talks.

The eurozone has been working to reach a voluntary agreement with banks, rather than forcing losses onto the lenders, because that could avoid triggering billions of euros on payout for bond insurance and could destabilize markets even further. However, in recent weeks some officials have no longer insisted that the deal remain voluntary.

Earlier Saturday, a European official said the EU was on track to agree on forcing banks to raise just over euro100 billion ($140 billion) to ensure they have enough cushion to weather further losses on their Greek bonds as well as market turmoil.

The official spoke on condition of anonymity because the discussions the deal was supposed to be unveiled by EU leaders at their summit Sunday.

"We have made real progress and have come to important decisions on strengthening European banks," George Osborne, Britain's chancellor of the exchequer, said as he left Saturday's meeting. Osborne did not say what the decision was.

Strengthening banks and slashing Greece's debts are critical to solving Europe's crisis, which is now threatening to engulf larger economies like Italy and Spain and is blamed for dampening growth across Europe and even the world.

The euro100 billion figure is likely to disappoint some analysts, although it was above recent press reports. A report by the International Monetary Fund has called for up to euro200 billion ($280 billion) to be poured into banks.

The new rules would force systemically important banks to raise their core capital ratios to 9 percent, compared with just 5 percent to 6 percent they needed to pass EU stress tests this summer. The ratio measures the amount of capital banks hold compared to their risky assets.

Despite that significant progress, agreement on arguably the most important measure has remained elusive to eurozone leaders: boosting the firepower of the currency union's euro440 billion ($600 billion) bailout fund to keep the crisis from spreading.

Increasing the effectiveness of the fund ? called the European Financial Stability Facility ? is meant to help prevent larger economies like Italy and Spain from being unable to afford to borrow money from markets. That's exactly what happened to Greece, Portugal and Ireland and why those three EU countries needed bailouts.

Germany and France still disagree over how to do that and failed to make much progress on that front Friday night. German Chancellor Angela Merkel and French President Nicolas Sarkozy are meeting Saturday evening in the hopes of moving toward a deal.

Ex-CEO surrenders to face overbilling charges (AP)

NEWARK, N.J. ? Federal prosecutors say the former president and CEO of a New Jersey-based company surrendered Thursday to face charges of overcharging the government for overseas reconstruction projects, including Afghanistan.

Derish Wolff faces a six-count indictment.

Prosecutors allege Wolff led a scheme to overbill the U.S. government in connection with hundreds of millions of dollars in overseas reconstruction projects over a nearly 20-year period.

Wolff once led international engineering consulting firm Louis Berger Group, which is based in Morristown. The contractor managed more than $1 billion in reconstruction contracts in Afghanistan.

The company issued a statement saying it has made substantial changes to its accounting practices and remains eligible for government contracts.

Wolff is scheduled to make an initial court appearance in Newark in the afternoon.

China vaccinates 4.5 million in fight against polio (Reuters)

HONG KONG (Reuters) ? China vaccinated 4.5 million children and young adults over the last five weeks in the western region of Xinjiang in a fight against polio after the disease paralyzed 17 people and killed one of them, the World Health Organization said.

Polio has broken out in China for the first time since 1999 and scientists say the strain originated from Pakistan. The outbreak marked the latest setback to a global campaign to eradicate polio, now endemic in only four countries -- Afghanistan, India, Pakistan and Nigeria.

"Even if they don't come down with any symptoms (carriers), by giving them polio vaccine we make that person less infectious," said Oliver Rosenbauer, WHO spokesman for the Global Polio Eradication Initiative in Geneva.

All 17 polio cases occurred in Hotan prefecture in the province of Xinjiang and the patients fell ill between early July and mid-September. The Geneva-based WHO assumes that for every case it finds, there would be 199 others infected with the virus without displaying symptoms, he added.

In large vaccination drives that started in early September, health workers have since vaccinated 4.5 million people with three doses each of the polio vaccine, the WHO said.

Patients and carriers of polio can shed the virus for up to eight weeks in their stools, and transmission occurs through contact with contaminated objects and sewage water.

"We are sensitizing disease surveillance in large hospitals to look for any child or adult displaying polio-like symptoms. We'll look out for new cases, we hope there won't be," he said in a telephone interview.

Polio causes flu-like symptoms such as fever, nausea and headache, and can result in paralysis within 24 to 72 hours. There is no cure for polio, and doctors only manage the symptoms.

CHALLENGE REMAINS

Rosenbauer said polio-free countries will be at risk of reinfection if they do not sustain high immunization rates and without strong political will to eradicate the disease.

"The rest of the world may be polio-free but because it is a communicable disease and people travel, polio-free countries can become reinfected ... this is what we are seeing in western China," Rosenbauer said.

There have been new 444 cases of polio globally so far this year, a quarter of those in Pakistan. There were 1,349 reported cases in 2010.

"Pakistan - it's our biggest problem in 2011," Rosenbauer said. "Just as China can become reinfected, so can polio-free areas in Pakistan become reinfected."

Eradication efforts are thwarted by conflict as well as logistical and technical problems.

"Vaccinating children during these immunization campaigns more often than not is compromised because of conflict and it is too dangerous for vaccination teams. They have to negotiate with local leaders to obtain safe passage," Rosenbauer said, referring to southern areas in Afghanistan and tribal areas in Pakistan.

UK inquiry finds former minister broke rules (AP)

LONDON ? Britain's departed defense minister Liam Fox breached rules by allowing a personal friend access to meetings and overseas visits ? including discussions with a U.S. military commander and senior Israeli officials, a report by the country's top civil servant ruled Tuesday.

Fox stepped down as defense secretary on Friday amid intense scrutiny over the role of Adam Werritty, who had posed as an aide, arranged private meetings and joined at least 18 trips overseas.

The former defense lobbyist, who was best man at Fox's wedding in 2005, had no official government role but appeared frequently at the minister's side across the globe.

In a report on an official inquiry, Cabinet Secretary Gus O'Donnell criticized Fox for allowing Werritty access to talks, failures to disclose links between his friend and political donors, and for sharing sensitive details of his travel schedule with his friend ? which had "posed a degree of security risk" to both Fox and ministry officials.

"The evidence in this report supports the conclusion of a clear breach of the ministerial code," O'Donnell wrote. "This was a failure of judgment on his part for which he has taken the ultimate responsibility in resigning office."

O'Donnell said Fox had ignored warnings from senior aides that his contacts with Werritty, who is in his early 30s and at least 15 years Fox's junior, were problematic.

The fact the minister had introduced Werritty to someone who later became a financial backer could also be seen by some as potentially wrong, the report said.

"In this case there was an inappropriate blurring of lines between official and personal relationships," O'Donnell wrote.

Fox, 50, was also criticized for failing to ensure ministry staff were present at a meeting with a potential supplier in Dubai ? which had been arranged by Werritty ? and at talks with senior Israeli officials. Official aides are required to attend such meetings under government rules.

However, O'Donnell confirmed that Fox did not compromise national security and had made no financial gain during his dealings with Werritty.

"I am pleased that the report makes clear that the two most serious allegations, namely of any financial gain sought, expected or received by myself and any breach of national security, have no basis," Fox said in a statement.

Jim Murphy, the main opposition Labour Party's defense spokesman, said the report had not fully addressed the extent of Werritty's role as a rogue adviser.

"This report only scratches the surface of potential wrongdoing. This is a murky business and it has not yet been resolved," he said.

O'Donnell confirmed that Werritty had previously distributed businesses cards describing himself as an aide to Fox, which he said likely gave the impression he represented Britain's government.

Werritty's "use of business cards portraying himself as an adviser ... risked creating the impression that Mr. Werritty spoke on behalf of the U.K. Government," O'Donnell said.

Opposition lawmakers had raised concerns that Werritty may have used his access to Fox to win influence or gain valuable contracts for his financial backers, though O'Donnell said both Werritty and Fox had insisted that the younger man had never lobbied the minister on behalf of clients.

But the report confirmed Werritty received funding from a not-for-profit organization called Pargav, which he told the inquiry supported work in the Middle East.

Donors to Pargav included a real estate company owned by the head of a lobbying organization which promotes ties between Israel and Britain, Michael Davis ? the chief executive of mining giant Xstrata ? and a risk management company with ties to Sri Lanka, the report said.

Another donor named in the report, Jon Moulton, provided evidence to the inquiry and said last week he had been approached by Fox to offer funds to Pargav.

Werritty should have not attended a meeting in September 2010 with Britain's incoming ambassador to Israel on "international defense and security matters," the report said.

O'Donnell called for "a more rigorous approach to avoid similar blurring of lines between personal and official business in the future."

Werritty had met with Fox on at least 40 occasions since the lawmaker took his post in May 2010, when Britain's current Conservative-led coalition government was established. He had also held meetings with two junior defense ministers.

In Washington, Pentagon press secretary George Little said Tuesday that Werritty had attended a March 2010 meeting between Fox ? then an opposition lawmaker ? and then-Defense Secretary Robert Gates. "It was a purely unclassified discussion," Little said in an email.

Werritty also joined in a steakhouse dinner in July 2010 in Tampa, Florida, with Fox and Lt. Gen. John R. Allen ? now the top U.S. commander in Afghanistan.

Prime Minister David Cameron's office said the British leader accepted the findings of the report and agreed that Fox had broken the ministerial code.

Cameron appointed Philip Hammond, previously Britain's Transport Secretary, to replace Fox in charge of defense.
